Why this matter is before the board: Approval from the Board of Directors is necessary to issue a Change Order to a constrcution contract. 2. Histoey: The Board previously awarded the Northside Transmission Pipeline projec. to Longo Incorporated. On July 7, 2004, the Board of Directors approved Change Order No. 1 for this Contract. That . Change Order covered the construction of about 730 feet of pipeline on High Street at cost of $81,282. On October 18, 2004, the Board of Directors approved Change Order No. 2 for this Contract. That Change Order covered the all outstanding issues encountered from the project start to September 30, 2004. 3. New information: I am requesting a Change Order to the Contract for issues covered three general items that will cover all of the outstanding issues encountered as of October 30, 2004. At this time, the project is complete except for paving. Item 1: Seven hours of rock excavation were necessary to install the pipeline on High Street. Longo is due $3,290.00 for this work. Item 2: There is an existing single family residence at 10132 Indian Jack Road. This house is located at about the same elevation as the Northside Tank. Consequently, there is not enough pressure to serve the house from the tank. The US Forest Service has a maintenance compound located on the adjacent property to the north. Water service is provided to the Forest Service by a pump in the Northside Well building that pumps to a tank at a higher elevation located on the Forest Service. This higher tank is owned and maintained by the Forest Service. Page 1 When the house was constructed, it was connected into the water system on the Forest Service property and not directly to a District-owned main. As described in a previous issue considered by then Board, The Forest Service is planning on closing this maintenance compound and selling the property. With the existing configuration, when the Forest Service maintenance compound is closed, water service to the residence at 10132 Indian Jack Road will be cut off. In order to maintain service to this customer, it is necessary to install a new service lateral connecting this house to the new Northside Transmission Pipeline. This pipeline is connected to the Bridge Street 6170 tank and has adequate pressure to service the residence. Longo was directed to install this new service lateral for the residence so that the destruction of newly installed paving would not be required next year. To date about 66% of the lateral has been installed and Longo is owed $9,405.74. Work on the lateral was been halted for the Winter and will be completed in the Spring of 2005. The Forest Service has indicated that the maintenance compound will remain open at least through the Fall of 2005. Item 3: The valve configuration shown on the original drawings for the High Street pipeline were based upon the existence of a valve on the south side of Donner Pass Road between the Roundabout and the railroad tracks. While arranging for the scheduled shutdown to connect the High Street pipeline to the existing piping in Donner Pase Road, District crews were unable to locate this valve. It appears that the valve wa paved over or buried during construction of the Roundabout and proper: a-s-lauih, drawings do not exist to locate the valve. Consequently, Longo was directed to install a new valve at the tie-in between the old and new pipe and is owed $5,521.33 for this work. 4. Recommendation: I recommend that the Board authorize Contract Change Order #3, in the amount of a $18,217.07 payment to Longo Incorporated from the District. The Contractor shall construct, furnish equipment and materials, and perform all work as necessary or required to complete the Change Order Items for a lump sum price agreed upon between the Contractor and District. The undersigned Contractor approves the foregoing Change Order as to the changes, if any, in the contract price specified for each item including any and all supervision costs and other miscellaneous costs relating to the change in work, and as to the extension of time allowed, if any, for completion of the entire work on account of said Change Order. The Contractor agrees to furnish all labor and materials and perform all other necessary work, inclusive of the directly or indirectly related to the approved time extension, required to complete the Change order items. This document will become a supplement of the contract and all provisions will apply hereto. It is understood that the Change Order shall be effective when approved by the District.
