Subject: Change Order No. 2 for the Northside Transmssion Pipeline Project
1. Why this matter is before the board:
Approval from the Board of Directors is necessary to issue a Change Order to a constrcution
contract.
2. History:
The Board previously awarded the Northside Transmission Pipeline project to Longo
Incorporated.
On July 7, 2004, the Board of Directors approved Change Order No. 1 for this Contract. That
Change Order covered the construction of about 730 feet of pipeline on High Street at a cost of
$81,282.
3. New information:
Construction of the project began in late June. 1 am requesting a Change Order to the Contract
for issues covered two general items that will cover all of the outstanding issues encountered as
of September 30, 2004.
Item 1: Rock removal requiring blasting was encountered at a few locations and Longo is
due $1,970 for this rock removal work.
Item 2: There are two flow control stations that are part of the project. These stations
consist of an underground vault that houses a control valve with piping that enters
and exits the vault, connect to the valve. Due to some differences between the
existing grades as shown on the drawings, and actual field conditions, it was
necessary to install a total of four vault extensions on these two vaults. Longo is due
$2,750 for purchase and installation of these vault extensions.
4. Recommendation:
I recommend that the Board authorize Contract Change Order #2, in the amount of a $4,720
payment to Longo Incorporated.

This Change Order covers changes to the subject Contract as described herein. The Contractor shall construct,
furnish equipment and materials, and perform all work as necessary or required to complete the Change Order
Items for a lump sum price agreed upon between the Contractor and District.

The amount of the Contract will be increased by the sum of $4,720.00 and the Contract Time shall not be
extended. The undersigned Contractor approves the foregoing Change Order as to the changes, if any, in the
contract price specified for each item including any and all supervision costs and other miscellaneous costs
relating to the change in work, and as to the extension of time allowed, if any, for completion of the entire work on
account of said Change Order. The Contractor agrees to furnish all labor and materials and perform all other
necessary work, inclusive of the directly or indirectly related to the approved time extension, required to complete
the Change order items. This document will become a supplement of the contract and all provisions will apply
hereto. It is understood that the Change Order shall be effective when approved by the District.
