Subject: Glenshire Pipelines -2003
1. WHY THIS MATTER IS BEFORE THE BOARD
The Glenshire Pipelines - 2003 Project is complete. It is time for the board to consider a
closeout change order and a resolution accepting the project.
2. HISTORY
The purpose of this project was to construct approximately 2,500 feet of 12-inch water
pipeline, approximately 300 feet of water service lateral, water meter boxes, fire hydrants
and laterals, connections to existing pipelines, and ancillary items such as fittings,
valves, marking tape and other appurtenances. The Glenshire Pipelines - 2003 Project
bid was awarded to FW Carson in the amount of$298,400 on July 8, 2003.
3. NEW INFORMATION
Attached for your review are the change order and the resolution.
4. RECOMMENDATION
a) Consideration of a contract change order Number 1 to the Glenshire Pipelines -
2003 Project
b) Consideration of a resolution accepting the Glenshire Pipelines - 2003 Project as
complete and authorizing the filing of the notice of completion.

BACKGROUND:
The Board of Directors previously awarded the Contract for construction of the
GLENSHIRE PIPELINES — 2003 project to FW Carson. The project was completed on
October 24, 2003, 1 am requesting a Change Order to the Contract for issues covered
under three general items that will cover all of the outstanding issues on this project.
Item 1: The contract was based on a unit price bid. The attached table shows the
differences between bid totals and the amounts actually constructed. This
analysis results in amount of $45,260 owed to FW Carson. Please note that
the additional cost is related entirely to pavement restoration issues.
Item 2: During construction of the project, two mismarked water pipes were
encountered and broken. The Contractor is owed $11,283.09 for extra work
related to the repair of these pipes and associated delays.
Item 3: There are a number of existing utilities located at the intersection of The
Strand and Donnington Lane (1-36" Storm Drain, 2-6" Water Pipes, 1-10"
Water Pipe and 1-2" Gas Pipe). The Contract drawings called for the new
pipe to be installed underneath all of these facilities. However, the existing
facilities were deeper than anticipated and the new pipeline had to be
installed about 2-3 feet deeper for a length of about 50 feet. The Contractor
is due $8,400 for extra work associated with this additional depth.
RECOMMENDATION:
I recommend that the Board authorize Contract Change Order #1, in the amount of a
$65,843.35 payment to FW Carson from the District.

The amount of the Contract will be increased by the sum of $64,943.09 and the Contract Time shall not be

rotating to the change In work, and as to the extension of time allowed, if any,for completion of the entire work on

AUTHORIZING THE ACCEPTANCE OF THE GLENSHIRE PIPELINES - 2003 AND
DIRECTING FILING OF THE NOTICE OF COMPLETION
WHEREAS, the Board of Directors of the Truckee Donner Public Utility District made the
determination to undertake the Glenshire Pipelines -2003 Project; and
WHEREAS, F.W. Carson Co., was selected by the District to perform the work; and
WHEREAS,the District's Water Systems Engineer, Neil Kaufman, has recommended that the work
be accepted by the District;
NOW THEREFORE BE IT RESOLVED by the Board of Directors of the District as follows:
1. That the District hereby accepts the Glenshire Pipelines - 2003 Project as complete.
2. That the Clerk of the District be directed to file with the County of Nevada the Notice of
Completion, a copy of which is attached hereto.
3. That 35 days following the filing of the Notice of Completion, the 10% retained from the
contractor payments be released to the contractor if no claims have been made to the District
by material suppliers or laborers.
