Subject: Final Completion and a Closeout Change Order for the Donner Lake
Pipelines - 2005 Project
1. Why this matter is before the board:
The Donner Lake Pipelines - 2005 is complete. A closeout change order and a resolution
accepting the project require Board approval.
2. History:
The Board previously awarded the Donner Lake Pipelines - 2005 project to Koch Excavating.
3. New information:
On November 4, District staff performed a final walkthrough on the project. All portions of the
project have been completed.
I am requesting a Closeout Change Order to the Contract for issues covered under 11 general
items that will cover all of the change order issues that arose during this project.
Item 1: The contract is based on a unit price bid. The attached table shows the differences
between bid totals and the amounts actually constructed. This analysis results in
amount of $68,631.00 owed to Koch Excavating. It should be noted that $64,952 of
this amount is related to rock excavation. Significantly more rock has been
encountered on the north shore of Donner Lake area compared with other areas of
Truckee. For comparison, the District Pipeline Replacement— 2005 project (which
will be presented to the Board later in November) has a difference between Bid and
Actual costs of$3,000 associated with rock excavation.
Item 2: On West Reed Avenue, the plans call for sliplining an existing 6" steel pipe with a
1.5" HDPE pipe. The steel pipe is quite shallow and is actually visible at the surface
at a couple of locations. It was decided to add some insulation to the 1.5 pipe to act
as additional protection against freezing. The District purchased the insulation, and
Koch is owed $462.00 for installation of the insulation.
Item 3: On West Reed Avenue, there is a flow control station that feeds water from the
Armstrong pressure zone to the Donner Lake 6124 pressure zone. The Water
Department is working towards installing a communications link between the flow

control station and the Wolfe Estates Tank. In order for the communications to
function properly, power and communications equipment must be installed in a
cabinet next to the flow control station vault. It was necessary for Koch to perform
some additional excavation and backfill work and install some additional conduit in
order to make connection to some existing conduits and to properly stub conduits for
the cabinet. Koch is owed $3463.90 for this work.
Item 4: The drawings call for Koch to install risers on an existing pole on West Reed Avenue.
During the course of the project, the Electrical Department replaced this pole
because it was in poor condition. SBC was not timely in relocating its telephone
facilities from the old pole to the new pole. As a result, Koch was forced to backfill
and then re-excavate part of the conduit trench in order to install risers on the pole.
Koch is owed $778.80 for this work.
Item 5: At 10095 Summit Drive, the plans call for sliplining an existing 6" steel pipe with a
1.5" HDPE pipe. Upon excavation, it was determined that the steel pipe was only
about 1-foot deep. It was decided to add some insulation to the 1.5" pipe to act as
additional protection against freezing. The District purchased the insulation, and
Koch is owed $308.00 for installation of the insulation.
Item 6: During the pressure testing and disinfection procedure for a new pipeline, new meter
boxes are tested to ensure proper flow. On Pioneer Drive, one meter box became
plugged and did not flow properly. Koch was directed to address the problem, after
flushing, it was determined that some dirt or debris had become lodged in the service
lateral and Koch excavated the lateral to remove the break. Upon excavation, Koch
found a piece of 12" PVC pipe had carried from somewhere else within the piping
system and become lodged inside the service. Koch did not install any 12" PVC and
the existing of the pipe fragment could not have been their fault. Therefore, I agreed
to reimburse Koch for the costs associated with fixing the problem. Koch is owed
$1,540.00 for this work.
Item 7: Along Donner Pass Road, the Drawings call for installation of a 3" HDPE pipe inside
an existing 4" welded steel pipe. Koch purchased 3" pipe for this purpose. During
initial efforts to install the 3" pipe, it was found that the interior weld beads on the 4"
pipe were large enough to prevent the 3" pipe from being inserted. It was decided to
change to a 2" pipe to solve the problem. The 2" pipe requires brass fittings instead
of plastic fittings. Consequently, material costs were higher and more labor was
required to assemble the pipe. Koch is owed $15,292.20 for this work.
Item 8: Along Donner Pass Road there were three locations where it was necessary to
excavate the existing steel pipeline in order to remove obstructions that prevented
pulling of the new pipe through the old pipe. These obstructions were two 45 degree
elbows and a coupling that were unknown to the District. Koch is owed $3,000.80 for
this work.
Item 9: Along Donner Avenue, there were also three locations where it was necessary to
excavate the existing steel pipeline in order to remove obstructions that prevented
pulling of the new pipe through the old pipe. These obstructions were two couplings
and a buried 6" valve that had been paved over in a driveway. Koch is owed
$3,517.25 for this work.

Item 10: At the driveway into the Donner Tract, Koch Excavating was to connect into an
existing pipe. There were two valves at this location and District personnel
mistakenly closed the wrong valve. As a result, the valve blew off the end of the pipe
creating a leak and flooding the excavation. Koch had to re-excavate the hole and
reinstall the valve. Koch is owed $1,076.16 for this work.
Item 11: The Truckee Sanitary District mismarked a sewer lateral Loch Leven Lodge. The
sewer lateral was subsequently damaged by Koch. Koch is owed $1,403.60 for
repairing this pipe.
Item 12: There were eight locations where it was necessary to install traffic rated meter
boxes. There was not a Bid Item in the Bid Schedule to cover this work. There were
six single meter installation and two double meter installations totaling $7,221.02.
4. Recommendation:
a) I recommend that the Board authorize Contract Change Order No. 1, in the amount
of a $106,694.73 payment to Koch Excavating from the District.
b) I recommend that the Board pass a Resolution accepting the Donner Lake
Pipelines — 2005 project as complete and authorizing the filing of the Notice of
Completion.

account of said Change Order. The Contractor agrees to furnish all labor and materials and perform all other

AUTHORIZING THE ACCEPTANCE OF
THE DONNER LAKE PIPELINES — 2005 PROJECT
AND DIRECTING FILING OF THE NOTICE OF COMPLETION
WHEREAS, the Board of Directors of the Truckee Donner Public Utility District made the determination to
undertake the Donner Lake Pipelines -2005 Project; and
WHEREAS, Koch Excavating was selected by the District to perform the work; and
WHEREAS, Koch Excavating has completed all of the Work included in the Project; and
WHEREAS, The District Engineer, Neil Kaufman, now certifies the completion of the project; and
WHEREAS,the District's Water Utility Manager, Ed Taylor,has recommended that the work be accepted by the
District;
NOW THEREFORE BE IT RESOLVED by the Board of Directors of the District as follows:
1. That the District hereby accepts the Donner Lake Pipelines - 2005 Project as complete.
2. That the Clerk of the District be directed to file with the County of Nevada the Notice of Completion, a
copy of which is attached hereto.
3. That 35 days following the filing of the Notice of Completion, the monies retained from the contractor
payments be released to the contractor if no claims have been made to the District by material suppliers or
laborers.

NOTICE IS HEREBY GIVEN:

2. Nature of interest or estate of owner:beneficial interest of a public utility easement for installation of water
facilities.
3. That on November 16'h, 2005 the hereinafter described improvements were accepted as complete
pursuant to Resolution 2005-XX of the Truckee Donner Public Utility District, the awarding authority.
4. That the subject improvements of the Notice are generally described and identified as follows:
