Subject: Final Completion and a Closeout Change Order for the Old Greenwood
Pump Development and Testing Project
1. WHY THIS MATTER IS BEFORE THE BOARD:
The Old Greenwood Pump Development and Testing project is essentially complete. A
closeout change order and a resolution accepting the project require Board approval.
2. HISTORY
The Board previously awarded the Old Greenwood Pump Development and Testing project
to Carson Pump. No project change orders have been previously brought to the Board for
approval.
3. NEW INFORMATION
The Old Greenwood Pump Development and Testing Project is now complete.
I am requesting a Closeout Change Order to the Contract for issues under 5 general items that
will cover all of the change order issues that arose during this project.
Item 1: Contract item, Conduct dual swab pump development, is based on hours. The
contract estimated 48 hours to accomplish this task. The project went faster than
normal and required 33 hours. This resulted in a 15 hour reduction in time or a
decrease in the amount of$4,125.00.
Item 2: Contract item, Furnish, install, and remove test pump, is based on per foot cost.
The contract estimated 400 feet of pump set to accomplish this task. The project
required an extra 20 feet of pump set. This resulted in an addition cost of
$280.00.
Item 3: Contract item, Conduct pump development program, is based on hours. The
contract estimated 24 hours to accomplish this task. The project went faster than

normal and required 3 hours. This resulted in a 21 hour reduction in time or a
decrease in the amount of$5,250,00.
Item 4: Contract item, Conduct Constant-Discharge Pumping Test, is based on hours.
The contract estimated 72 hours to accomplish this task. The project went faster
and better than normal and required 58 hours. This resulted in a 14 hour
reduction in time or a decrease in the amount of$3,360.00.
Item 5: This item was extra work. We discovered debris in the bottom 22 feet of the well.
Carson Pump was requested to remove this debris. This resulted in an increase
amount of$975.00.
4. RECOMMENDATION
a) I recommend that the Board authorize Contract Change Order#1, In the amount of an
$11,480.00 deduction of the contract amount to Carson Pump.
b) I recommend that the Board pass a Resolution accepting the Old Greenwood Pump
Development and Testing as complete and authorizing the filing of the notice of
completion.

This Change Order covers changes to the subject Contract as described herein. The Contractor shall construct,
furnish equipment and materials, and perform all work as necessary or required to complete the Change Order
Items for a lump sum price agreed upon between the Contractor and District.

The amount of the Contract will be decreased by the sum of -$11,480.00 and the Contract Time shall not be
extended. The undersigned Contractor approves the foregoing Change Order as to the changes, if any, in the
contract price specified for each item including any and all supervision costs and other miscellaneous costs
relating to the change in work, and as to the extension of time allowed, if any, for completion of the entire work on
account of said Change Order. The Contractor agrees to furnish all labor and materials and perform all other
necessary work, inclusive of the directly or indirectly related to the approved time extension, required to complete
the Change order items. This document will become a supplement of the contract and all provisions will apply
hereto. It is understood that the Change Order shall be effective when approved by the District.

AUTHORIZING THE ACCEPTANCE OF
PUMP DEVELOPMENT AND TESTING PROGRAM FOR THE OLD
GREENWOOD PRODUCTION WELL PROJECT
AND DIRECTING FILING OF THE NOTICE OF COMPLETION
WHEREAS, the Board of Directors of the Truckee Donner Public Utility District made the determination to
undertake the Pump Development and Testing Program for the Old Greenwood Production Well Project; and
WHEREAS, Carson Pump was selected by the District to perform the work; and
WHEREAS, Carson Pump has completed all of the Work included in the Project; and
WHEREAS, The District's Consultant, David Carlson of AquaHydrogeologic Consulting, now certifies the
completion of the project; and
WHEREAS,the District's Water Utility Manager, Ed Taylor, has recommended that the work be accepted bythe
District;
NOW THEREFORE BE IT RESOLVED by the Board of Directors of the District as follows:
I. That the District hereby accepts the Pump Development and Testing Program for the Old Greenwood
Production Well Project as complete.
2. That the Clerk of the District be directed to file with the County of Nevada the Notice of Completion, a
copy of which is attached hereto.
3. That 35 days following the filing of the Notice of Completion, the monies retained from the contractor
payments be released to the contractor if no claims have been made to the District by material suppliers or
laborers.

NOTICE IS HEREBY GIVEN:
1. That the name and address of the public entity for whom the improvement project was done, as owner
thereof, is the Truckee Donner Public Utility District, Post Office Box 309, Truckee, CA 96160.
2. Nature of interest or estate of owner: beneficial interest of a public utility easement for installation of water
facilities.
3. That on November 161", 2005 the hereinafter described improvements were accepted as complete
pursuant to Resolution 2005-XX of the Truckee Donner Public Utility District, the awarding authority.
4. That the subject improvements of the Notice are generally described and identified as follows:
Pump Development & Testing for the Old Greenwood Production Well.
5. That the name and address of the contractor for such project was: Carson Pump, P.O. Box 20159,
Carson City, NV 89721.
