Why this matter is before the board: Change Orders to construction contracts require Board approval. 2. History: The Board previously awarded the Donner Lake Pipelines-2004 project to Jurin Construction. On August 4, 2004, the Board approved Change Order No. 1 that covered all outstanding issues from the project start through June 2, 2004. On August 18, 2004, the Board approved Change Order No. 2 that covered all outstanding issues from June 3 through August 6, 2004. On September 15, 2004, the Board approved Change Order No. 3 that covered all outstanding issues from August 7 through September 3, 2004. On October 18, 2004, the Board approved Change Order No. 4 that covered all outstanding issues from September 4 through October 10, 2004. On December 15, 2004, the Board approved Change Order No. 5 that covered all outstanding issues from October 11 through November 12, 2004. On July 20, 2005, the Board approved Change Order No. 6 that closed out the remaining financial issues associated with the project. The Board also accepted the project as complete. 3. New information: While preparing the documentation to process a payment to Jurin Construction based upon the change order, an error in the analysis of Bid vs. Actual quantities was discovered. Change Order No. 6 presented at the July 20 Board Meeting showed that Jurin Construction was owed $31,650.80 for this issue. The correct amount is $42,450.80. Therefore, I have a prepared a new Change Order to correct this error. t Page 1 z 4. Recommendation: I recommend that the Board authorize Contract Change Order No. 7, in the amount of a $10,800.00 payment to Jurin Construction from the District. The Contractor shall construct, furnish equipment and materials, and perform all work as necessary or required to complete the Change Order Items for a lump sum price agreed upon between the Contractor and District. The undersigned Contractor approves the foregoing Change Order as to the changes, if any, in the contract price specified for each item including any and all supervision costs and other miscellaneous costs relating to the change in work, and as to the extension of time allowed, if any, for completion of the entire work on account of said Change Order. The Contractor agrees to furnish all labor and materials and perform all other necessary work, inclusive of the directly or indirectly related to the approved time extension, required to complete the Change order items. This document will become a supplement of the contract and all provisions will apply hereto. It is understood that the Change Order shall be effective when approved by the District.
