WHY THIS MATTER IS BEFORE THE BOARD Board approval is required for expenditures in excess of $15,000. 2. HISTORY At the April 15, 2015 meeting, the Board awarded a contract in the amount of $21,800, plus a 10% contingency, for an amount not to exceed $23,980 to Carson Pump, LLC for the Airport Well Maintenance 2015. The contract included the removal and inspection of the well pump assembly and electric motor, installation of a new pump shaft assembly, video inspection of the well and axial alignment survey of the well column. 3. NEW INFORMATION Following the inspection of the Airport Well motor and pump assembly, staff has reviewed the inspection report and has detemrined the need to perform maintenance on the 250HP electric pump motor including reconditioning motor stator, balance rotor and replace bearings. Additionally, following the review of the video inspection of the well column, staff has determined the need to brush the well column and perforations in an effort to maintain well performance and productivity. The cost associated with the additional labor and materials will require a change order in the amount of $4,753. IM The change order adds an additional $4,753 to the original contract, bringing the total amount of the contract to $28,733. Sufficient funds exist within the approved FY15 Water Operations Budget for this contract. 5. RECOMMENDATION Authorize Change Order #1 to the Airport Well Maintenance 2015 contract in the amount of $4,753 for a new contract total amount of $28,733, Michael D.
