NOCAgendaItem#r7‘.TRUCKEEDONNERPublicUtilityDistrictCONSENTTo:BoardofDirectorsFrom:BrianWrightDate:June19,2019Subject:ConsiderationtoAdoptResolution2019-19toClose-OuttheOldGreenwoodWellRehabilitationProject_____________________1WHYTHISMATTERISBEFORETHEBOARDTheDistrict’sOldGreenwoodWellRehabilitationProjectiscomplete.AresolutionacceptingtheprojectascompleteandfilingofthenoticeofcompletionrequiresBoardapproval.2.HISTORYInSeptember,2018theDistrict’sOldGreenwoodWellexperiencedamechanicalfailure.Staffdeterminedthefailuretobesignificantenoughtorequiretheremovalandinspectionofthewellpumpandmotorassemblybyalicensedwellconstructionandmaintenancecontractor.InOctober,2018theDistrictenteredintoaconstructionservicesagreementwithCarsonPumptoremovetheOldGreenwoodWellpumpandmotorassemblyforinspectionandevaluation.Followingtheremovalandinspectionofthepumpandmotorassembly,thecauseofthefailurewasidentifiedinthewelllineshaftbearingandcouplingassemblies,causingsignificantdamagetothelineshaftandpumpdischargepipe.Staffevaluatedavailablereplacementoptions,anddeterminedthemostbeneficialoptiontotheDistrictwouldbetoreplacetheexisting12”diameterwelldischargeandlineshaftassemblywithanew8”diameterassembly.Thisoptionwouldalsoincludetheinspectionandmaintenanceoftheexistingpumpbowlassemblytoensuremanufacturerperformancespecificationsandmodificationofthewelldischargeheadtomoreaccuratelyaccommodatethewell’shistoricgroundwaterproductionperformance.InJanuary,2019theDistrictpubliclyadvertisedarequestforbidsfortheOldGreenwoodWellRehabilitationProject.OnJanuary25,2019apublicbidopeningwasheld,atwhichtimetheDistrictreceivedtworesponsivebids.Followingthereviewofbids,CommercialPumpandMechanicalwasdeterminedtobethelowestresponsivebidderwithatotalbidamountof$105,417.TheBoardauthorizedtheGeneralManagertoexecuteaconstructionservicescontractforatotalamountnottoexceed$116,000withCommercialPumpandMechanicalfortheOldGreenwoodWellRehabilitationProjectattheFebruary6,2019regularlyscheduledmeeting. 3.NEWINFORMATIONInMay,2019,CommercialPumpandMechanicalcompletedtheworkincludedintheOldGreenwoodWellRehabilitationProjectasprescribedintheconstructioncontractagreement.StaffhasconductedoperationaltestingofthewelltoverifytheperformancemeetstheDistrict’sspecificationsandstandards.Thewellhasbeenreturnedtoservice.OnJune6,2019,theDistrictconductedafinalinspectionoftheworkperformedfortheOldGreenwoodWellRehabilitationProject.TheworkperformediscompleteandreadytobeacceptedbytheDistrict.Attachedisresolution2019-19,acceptingthe4.FISCALIMPACTTheapprovedcontractpricewas$105,417,plusa10%changeorderauthorization,foratotalnottoexceedamountof$116,000.ThisprojectwasnotincludedintheBoardapprovedFYi9WaterUtilityOperationsBudget.SufficientfundsexistwithintheWaterOperatingReserves.5.RECOMMENDATIONAdoptResolution2019-19acceptingtheOldGreenwoodWellRehabilitationProjectascompleteandauthorizethefilingoftheNoticeofCompletion.frzMichaelD.HolleyGeneralManagerIWaterUtilityDirector
