WHY THIS MATTER IS BEFORE THE BOARD The District Pipeline Replacement - 2015 project is complete. A resolution accepting the project as complete requires Board approval. 2. HISTORY In April 2015, the Board awarded the construction contract for the District Pipeline Replacement - 2015 project to AM-X Construction & Excavation (AMX) of Truckee, California. The project involved replacement of about 4,900 feet of pipeline in three segments along Northwoods Boulevard in Truckee. The project also included installation of about 7,850 feet of fiber optic inner duct. AMX began work in June and completed the construction at the end of October. The original bid price was $1,478,273.75. At the time of award, the Board also established a 10 percent change order allowance for a total for a total authorization not to exceed $1,626,000. 3. NEW INFORMATION During the course of the project, 15 change order requests were submitted and reviewed by staff. After review and discussion, the District and AMX agreed on a total of $25,238.22 for these issues. This amount did not exceed the $147,726.25 change order allowance established by the Board of Directors. The items generally fell under the following categories: • Additional work such as installation of fire hydrant extensions and AC dike • Repair of water leaks • Unmarked or mis-marked utilities There was one item that resulted in a credit to the District. The contract is based on a unit price bid. Attachment 1 shows the differences between bid totals and the amounts actually constructed. This analysis results in a $47,597.29 credit to the District. Page 1 of 5 In mid -October, District staff performed a final walkthrough on the project. The work performed is complete and should be accepted by the District. Attached is a draft resolution (Attachment 2) accepting the project and directing that a Notice of Completion be filed. !11111126W-11aIIT, 1:7_1;O1 The original bid price was $1,478,273.75. Once adjustments are made for the items described above, the final contract price is $1,455,914.68. As discussed at the time of award, the anticipated funding sources for the project were: Funding Source Amount Electric Department General Fund Water Department Capital Replacement Fund $161,723.75 $1,316,550.00 Total $1,478,273.75 After adjustments for change orders and the reconciliation of bid vs. actual quantities, the final funding for the project is: Funding Source Amount Electric Department General Fund Water Department Capital Replacement Fund $166,139.95 $1,289,774.73 Total $1,455,914.68 5. Holley, has recommended that the work be accepted by the District; NOW THEREFORE BE IT RESOLVED by the Board of Directors of the District as follows: 1. That the District hereby accepts the District Pipeline Replacement - 2015 Project as complete. 2. That the Clerk of the District be directed to file with the County of Nevada the Notice of Completion, a copy of which is attached hereto. 3. That 35 days following the filing of the Notice of Completion, the monies retained from the contractor payments be released to the contractor if no claims have been made to the District by material suppliers, laborers, or sub -contractors.
