WHY THIS MATTER IS BEFORE THE BOARD The Fibreboard Well Drilling Pump Development and Testing Project is complete. A closeout change order and a resolution accepting the project require Board approval. 2. HISTORY The Board previously awarded the Fibreboard Well Drilling Pump Development & Testing program to Carson Pump, LLC in an amount not to exceed $66,202.50 (includes 5% original change order authorization). 3. NEW INFORMATION Well development and testing began in November 2008 after the well drilling was complete. As of December 2008 all portions of the pump development are complete. Staff is now requesting Board authority to file a formal Notice of Completion. A closeout change order is requested for this Contract. The changes to the contract include: • An increase of $14,000 for the use of a drilling mud dispersant compound and seven extra hours of test pumping • A decrease of $9,425 due to a reduction of 20 hours of development pumping and 2.5 hours of step draw down pumping These changes result in a net increase of $4,575 and a revised Contract total of $67,625. 4. FISCAL IMPACT A change order in the amount of $4,575.00 will bring the new contract total to $67,625.00, which exceeds the total approved contract amount of $66,202.50 (includes 5% original change order authorization). Sufficient funds exist in facilities fees to fund this change order. 5. RECOMMENDATION a) Authorize Contract Change Order No. 1, in the amount of$4,575.00 to Carson Pump LLC b) Authorize the transfer of$4,575 from facilities fees to the Water General Fund c) Adopt the attached draft resolution accepting the Fibreboard Well Drilling Pump Development and Testing program as complete and authorize staff to file the Notice of Completion Ed Taylor Michael D. The undersigned Contractor approves the foregoing Change Order as to the changes, if any, in the contract price specified for each item including any and all supervision costs and other miscellaneous costs relating to the change In work, and as to the extension of time allowed, if any,.for completion of the entire work on account of said Change Order. The Contractor agrees to furnish all labor and materials and perform all other necessary work, inclusive of the directly or indirectly related to the approved time extension,required to complete the Change order items. This document will become a supplement of the contract and all provisions will apply hereto. It is understood that the Change Order Shall be effective when approved by the District_ Accepted: /Ccttitrector F3ata: / (Signature) Approved: /District Date: (Signature) Page 1 k CKEE r DONNER �- PLAAC Utility District Resolution No. 2009 - XX AUTHORIZING THE ACCEPTANCE OF THE FIBREBOARD PRODUCTION WELL PUMP DEVELOPMENT & TESTING PROGRAM AND DIRECTING FILING OF THE NOTICE OF COMPLETION WHEREAS, the Board of Directors of the Truckee Donner Public Utility District made the determination to undertake the Fibreboard Production Well Pump Development& Testing Program; and WHEREAS, Carson Pump, LLC was selected by the District to perform the work; and WHEREAS, Carson Pump, LLC has completed all of the Work included in the Project; and WHEREAS, The District's Consultant, David Carlson of Aqua Hyd rogeolog ic Consulting, now certifies the completion of the project; and WHEREAS,the District's Water Utility Manager, Ed Taylor, has recommended that the work be accepted bythe District; NOW THEREFORE BE IT RESOLVED by the Board of Directors of the District as follows: 1. That the District hereby accepts the Fibreboard Production Well Pump Development&Testing Program as complete. 2. That the Clerk of the District be directed to file with the County of Nevada the Notice of Completion, a copy of which is attached hereto. 3. That 35 days following the filing of the Notice of Completion, the monies retained from the contractor payments be released to the contractor if no claims have been made to the District by material suppliers or laborers.
