BACKGROUND:
In the fall of 200, the Board of Directors awarded the Contract for the INTERSTATE 80
PIPELINE REHABILITATION project to Hydrotech Incorporated. The project has been
completed and I am requesting a Change Order covering 2 items,which represent the only
outstanding issues for this contract .
Item 1: The original Contract indicated that all earthwork and excavations would be
performed by the District. There were concerns related to stability of the
excavations and ii was decided that temporary shoring should be utilized.
The cost of this item ($2,052.75) represents the Contractor's labor involved
in installation of the shoring.
Item 2: There is a valve assembly located exposed and aboveground adjacent to the
excavation on the south side of Donner Creek. One of these valves froze
and split causing a leak than washed dirt and debris into the excavation and
the newly lined pipe. This total ($2,052.75) represents costs involved
removing debris from the excavation and use of a vacuum cleaning truck to
remove debris from inside of the pipe.
RECOMMENDATION:
I recommend that the Board authorize Contract Change Order #1 in the amount of
$4,205.55 with no change in the Contract completion date.

This Change Order covers changes to the subject Contract as described herein. The Contractor shall construct,
furnish equipment and materials, and perform all work as necessary or required to complete the Change Order
Items for a lump sum price agreed upon between the Contractor and District.

The amount of the Contract will be increased (decreased) by the sum of$4,205.55 and the Contract Time shall not
be extended. The undersigned Contractor approves the foregoing Change Order as to the changes, if any, in the
contract prim specified for each item including any and all supervision costs and other miscellaneous costs relating
to the change in work, and as to the extension of time allowed, if any, for completion of the entire work on account
of said Change Order, The Contractor agrees to furnish all labor and materials and perform all other necessary
work, inclusive of the directly or indirectly related to the approved time extension, required to complete the Change
order items. This document will become a supplement of the contract and all provisions will apply hereto. It is
understood that the Change Order shall be effective when approved try the District.

AUTHORIZING THE ACCEPTANCE OF THE INTERSTATE 80 PIPELINE REHABILITATION PROJECT
AND DIRECTING FILING OF THE NOTICE OF COMPLETION
WHEREAS, the Board of Directors of the Truckee Donner Public Utility District made the
determination to rehabilitate the Interstate 80 pipeline; and
WHEREAS, the contract was awarded to Hydrotech, Inc., and the contractor entered into an
agreement with the District to perform the work; and
WHEREAS, the District's Water Engineer, Neil Kaufman, has recommended that the work be
accepted by the District;
NOW THEREFORE BE IT RESOLVED by the Board of Directors of the District as follows:
1. That the District hereby accepts the Interstate 80 Pipeline Rehabilitation Project as complete.
2. That the Clerk of the District be directed to file with the County of Nevada the Notice of
Completion, a copy of which is attached hereto.
3. That 35 days following the filing of the Notice of Completion, the 10% retained from the
contractor payments be released to the contractor if no claims have been made to the District by
materiel suppliers or laborers.

NOTICE IS HEREBY GIVEN:
1. That the name and address of the public entity for whom the improvement project was done, as owner
thereof, is the TRUCKEE DONNER PUBLIC UTILITY DISTRICT, Post Office Box 309, Truckee, CA 96160.
2. That on February 20, 2001, the hereinafter described improvements were accepted as complete pursuant
to a resolution of the Truckee Donner Public Utility District, the awarding authority.
3. That the subject improvements of the Notice are generally described and identified as follows: Interstate
80 Pipeline Rehabilitation.
4. That the name of the contractor for such project was: Hydrotech, Inc., 155 Lyon Drive, Fernley, NV
89408.
