MEMORANDUM

SUBJECT: BRIDGE STREET 6160 WATER TANK
With this memo is Contract Change Order No. 2 for the Bridge Street 6160 Water Tank.
This is the final close-out change order for the project. Contract Change Order No. 2 is for
an increase of$106,745.55 and a time extension of 30 days. This represents an increase of
8% over the original contract price. Also included is the Resolution of Acceptance and
Notice of Completion for the project.
Item I
Construction of additional pipeline. The original contract had the tank contractor terminating
the pipeline at the end of the new tank access road. At the same time, the contractor for the
nearby Pioneer Village project extended a distribution main to within 340 feet of the tank
access road. This item included the additional 340 feet of pipeline to connect the tank to the
distribution system and allow the tank to be put in service.
Item 2
Rock excavation. During the construction of the tank pad and cut slope,the contractor
encountered rock which required removal by blasting or other specialized rock breaking
equipment. The contractor opted for the use of extra heavy duty rock breaking equipment as
a less expensive alternative to blasting. This item is for the utilization of the rock breaking
equipment.
Item 3
Additional erosion control measures. In order to obtain the Lahontan RWQCB permit for
construction of the tank project, additional erosion control measures were added to the
project. These included additional hydroseeding, construction of rock lined V-ditches along
the access road, and construction of road cross drains and rock lined detention basins along
the access road. This item is for these additional measures.
Item 4
Delete gunite on detention basin embankment. The original contract required the application
of"Shotcrete" gunite on the interior face of the detention basin embankment. This was

included in the design because the material to be used for the embankment was
expected to be mostly porous, light-weight red cinder material from the tank pad
excavation. As constructed,the material used for the embankment was well mixed
including cinders, silts, clay, and soil. It was determined that this material did not
require a gunite face. This item is a credit for the deletion of the gunite.
Item 5
Time extension. The contractor did proceed with due diligence through the course of
the project. The tank was completed on schedule, however some miscellaneous items
remained to be completed. This item is for a time extension for the completion to the
project.
RECOMMENDATION
I recommend the Board of Directors take the following actions:
I. Approve Contract Change Order No. 2 for the Bridge Street 6160 Water Tank
for an increase of$106,745.55 and a time extension of 30 days.
2. Adopt the Resolution of Acceptance and authorize the filing of the Notice of
Completion.

AUTHORIZING THE ACCEPTANCE OF THE BRIDGE STREET 6160 TANK PROJECT AND
DIRECTING FILING OF THE NOTICE OF COMPLETION
WHEREAS, the Board of Directors of the Truckee Donner Public Utility District made the
determination to undertake the Bridge Street 6160 Tank Project; and
WHEREAS, CB &I Water, was selected by the District to perform the work; and
WHEREAS, The District's Consulting Engineer and Engineer of Record, Keith Knibb, certifies
the completion of the project; and
WHEREAS, the District's Water Utility Manager, Ed Taylor, has recommended that the work be
accepted by the District;
NOW THEREFORE BE IT RESOLVED by the Board of Directors of the District as follows:
1 That the District hereby accepts Bridge Street 6160 Tank Project as complete.
2 That the Clerk of the District be directed to file with the County of Nevada the Notice of
Completion, a copy of which is attached hereto.
3 That 35 days following the filing of the Notice of Completion, the 10% retained from the
contractor payments be released to the contractor if no claims have been made to the
District by material suppliers or laborers.
